Rental Contract

TERMS: Net 10th Prox.

SERVICE CHARGE: 11/2% per month on unpaid balance over 30 days.(APR 18%).

RENTEE IS RESPONSIBLE FOR THEFT of equipment. KEEP IT LOCKED!

RENTEE IS RESPONSIBLE FOR ALL DAMAGE TO TIRES and tubes caused by blowout, bruises, cuts, road hazards or other causes inherent in the use of the equipment.

RENTEE MUST CARRY A LIABILITY INSURANCE in the amount of 1 Million Dollars or greater and name BZ Equipment as additional insured.

RENTEE MUST CHECK OIL AND WATER DAILY.

CLEANING CHARGES are applicable to equipment returned unclean. MINIMUM CLEANING CHARGE $ __________

Renter leases to the undersigned Rentee the above-described equipment upon the terms and conditions set forth on this page and on the back hereof. Rentee promises to pay all charges provided here in and to return the above described equipment in the manner as set forth above. Renter makes no warranties as to equipment returned as set forth above. Renter makes no warranties as to equipment's merchant ability of fitness for any particular purpose.

California Vehicle Code Section 26709 requires a second rear view mirror to be located on the right hand side of the vehicle if the trailer or the load thereon obstructs the driver's view to the rear.

Customer is Responsible for REFUELING and DAMAGES to TIRES

ACCIDENTAL DAMAGE WAIVER:

For an additional fee, BZ agrees to waive claims for damage to the rented items as specified in this contract. Damage Waiver is required unless customer has previously supplied BZ with a certificate of insurance showing protection is provided BZ for any damage to the rented items.

Customer understands the damage waiver is not insurance.

I have been given safety instructions before operating or allowing rented equipment to be operated or used.

IF I DO NOT UNDERSTAND, OR FORGET THE SAFETY/OPERATING INSTRUCTIONS I HAVE BEEN GIVEN, OR IF THE EQUIPMENT FAILS, I WILL NOT ATTEMPT TO OPERATE OR REPAIR IT. I WILL DISCONTINUE USE AND NOTIFY BZ EQUIPMENT IMMEDIATELY.
